Addressing the Overburden on Healthcare Workers Due to the Influx of Undocumented Migrants in South Africa

We, the undersigned nurses, healthcare professionals, and concerned individuals working within the healthcare sector in South Africa, hereby express our profound concern and growing distress regarding the unsustainable strain placed upon our healthcare system due to the increasing number of undocumented migrants seeking medical care.
Our commitment to providing quality healthcare to all individuals within our facilities, in accordance with ethical guidelines, is unwavering. However, the current situation has led to an unprecedented overburdening of our already stretched resources, including:
Increased Patient Load: The significant influx of patients who are not documented residents places an additional and often unmanageable demand on our time, capacity, and resources within healthcare facilities across South Africa.
Resource Depletion: Limited medical supplies, essential equipment, and the availability of adequately staffed personnel are being further strained. This depletion impacts our ability to provide timely and effective care to all patients, including South African citizens and documented residents who contribute to the healthcare system.
Financial Strain: The costs associated with providing healthcare to individuals who may not contribute to the tax base or have the means to pay for services are escalating. This places a significant and growing burden on the public healthcare system's already limited budget, potentially diverting funds from essential services for the broader population.
Compromised Quality of Care: The overwhelming workload, coupled with resource limitations, is inevitably compromising the quality of care that healthcare professionals can provide to all patients. This can lead to longer waiting times, reduced attention per patient, and potentially adverse health outcomes.
Increased Stress and Burnout: Healthcare workers, who are already operating under demanding conditions, are experiencing heightened levels of stress, fatigue, and burnout due to the excessive demands placed upon them. This impacts their well-being, morale, and the long-term sustainability of the healthcare workforce in South Africa.
Language and Cultural Barriers: Communication challenges and the need to navigate diverse cultural backgrounds can further complicate the provision of effective and culturally sensitive care, adding to the workload and demands on healthcare professionals.
We recognize the complexities surrounding migration and the humanitarian needs of all individuals. However, the current situation is not sustainable and requires urgent attention and effective solutions within the South African context.
Therefore, we, the undersigned, respectfully request the relevant authorities to take immediate and decisive action to:
1. Strengthen border management and immigration control measures to manage the influx of undocumented migrants effectively, while upholding human rights principles.
2. Review and clarify policies regarding access to healthcare for undocumented migrants in South Africa to ensure clarity for both healthcare providers and those seeking care, and to ensure the sustainability of the system.
3. Allocate adequate and ring-fenced resources to address the increased demand on the healthcare system specifically related to the provision of care for undocumented migrants, without negatively impacting the resources available for South African citizens and documented residents. This may include specific funding mechanisms or dedicated facilities.
4. Develop and implement efficient systems for the accurate collection of data on healthcare utilization by undocumented migrants to better understand the scale of the impact and inform policy decisions.
5. Provide additional support and resources for healthcare workers, including increased staffing, language services, and cultural sensitivity training, to help them manage the increased demands and complexities of their roles.
6. Foster inter-departmental collaboration between the Department of Health, the Department of Home Affairs, and other relevant entities to develop a comprehensive and sustainable approach to managing healthcare provision in the context of migration.
